Structural Heart Fellowship Programme

2026/10/13

Structural heart disease is becoming increasingly common with our aging population. Rapid advances in percutaneous structural heart intervention, new device technologies, clinical trials, advanced imaging, and interventional techniques are reshaping the way these conditions are diagnosed and treated. Expertise in structural heart disease will therefore become an increasingly essential component of comprehensive modern cardiac care.

At the same time, advances in paediatric and congenital heart disease care have enabled more patients to reach adulthood, driving a growing need for specialised Adult Congenital Heart Disease (ACHD) services. The management of ACHD requires a comprehensive approach that combines lifelong clinical follow-up, advanced imaging, and, where appropriate, minimally invasive catheter-based interventions such as Atrial Septal Defect (ASD) or Patent Foramen Ovale (PFO) closure and transcatheter valve therapies.

Together, Structural Heart Disease and ACHD represent an important and rapidly evolving component of the future of cardiovascular medicine. They bring together clinical expertise, advanced imaging, multidisciplinary care and collaboration, and innovative minimally invasive therapies to improve outcomes for patients with complex cardiovascular conditions.

What is this programme about?

The Structural Heart Fellowship Programme at the National University Heart Centre, Singapore (NUHCS) is designed to provide comprehensive subspecialty training in the management of structural heart disease and Adult Congenital Heart Disease (ACHD).

Fellows will gain hands-on exposure to the full continuum of care for patients with complex structural heart conditions, from clinical assessment and diagnosis to advanced cardiac imaging, procedural planning, and catheter-based interventions.

The fellowship programme combines clinical expertise, advanced imaging, multidisciplinary collaboration, and hands-on procedural training in the treatment of complex cardiovascular conditions.

What are the key areas of focus?

The fellowship offers broad exposure across structural heart disease and ACHD, with training focused on:

  • Clinical assessment and management of structural heart disease and ACHD patients.
  • Advanced cardiac imaging, including transthoracic echocardiography, Transoesophageal Echocardiography (TEE) and Intracardiac Echocardiography (ICE).
  • Haemodynamic assessment and right heart catheterisation.
  • Catheter-based structural heart interventions, including:
    • Transcatheter Aortic Valve Implantation (TAVI)
    • Mitral and Tricuspid Transcatheter Edge-to-Edge Repair (TEER)
    • Transcatheter Valve-in-Valve (ViV) procedures
    • Pulmonary valve implantation
    • ASD and PFO closure.
    • Percutaneous Transvenous Mitral Commissurotomy (PTMC)
    • Left Atrial Appendage (LAA) closure
    • Catheter-based pulmonary embolism interventions and pulmonary embolectomy.

Who is this programme for?

The programme is intended for qualified cardiologists who wish to pursue subspecialty training in structural heart disease and Adult Congenital Heart Disease. Applicants should be accredited in General Cardiology, with prior experience in interventional cardiology, echocardiography, and transoesophageal echocardiography (TEE) imaging being advantageous.

The fellowship is suited for cardiologists seeking to develop advanced clinical, imaging, procedural planning, and interventional skills in the comprehensive management of complex structural heart conditions.

What are the key learning opportunities for fellows?

The fellowship programme provides comprehensive training across all aspects of structural heart disease and Adult Congenital Heart Disease, with progressive development of clinical, imaging, procedural, and research skills.

Clinical and Imaging Training

  • Comprehensive assessment and diagnosis of patients with structural heart disease.
  • Interpretation of complex transthoracic and Transoesophageal Echocardiography (TEE) imaging.
  • Training in Intracardiac Echocardiography (ICE) and its application during structural interventions.
  • Performance and interpretation of right heart catheterisation and haemodynamic assessment.
  • Supervised structural heart clinics, including new referrals, post-procedure follow-up, and management of complex structural heart conditions.
  • Assessment and management of inpatient referrals, ranging from stable patients to urgent and critically-ill patients.

Interventional Training

  • Hands-on experience in the cardiac catheterisation laboratory and operating theatre.
  • Exposure to a wide range of structural heart and ACHD procedures.
  • Procedural planning and peri-procedural management of complex cases.

Multidisciplinary and Academic Training

  • Participation in multidisciplinary Structural Heart Team meetings, including intra- and inter-hospital case discussions.
  • Opportunities to engage in clinical research, prospective registries, and academic projects.
  • Opportunities to present clinical cases and research findings at regional and international conferences.
  • Exposure to the development of contemporary structural heart programmes, including multidisciplinary decision-making and complex procedural planning.

Programme highlights
The Structural Heart Fellowship Programme at the National University Heart Centre, Singapore (NUHCS) is one of the highest-volume training hubs in the region. What makes it unique is its broad integration with specialised services including Adult Congenital Heart Disease (ACHD), the Women's Heart Health Programme, and the Pulmonary Embolism Response Team (PERT). Fellows will be exposed to cutting-edge technologies, including NUHCS's pioneering use of Intracardiac Echocardiography (ICE) to perform procedures safely without general anaesthesia or TEE. With active clinical research, registry participation, and academic development available, the fellowship programme offers complete preparation for both clinical mastery and academic development.
Length of fellowship
The Structural Heart Fellowship Programme is offered as a one-year fellowship. Fellows will be awarded a Certificate upon successful completion of the programme.
